Entering adulthood is a HUGE step – for both children and their parents/caregivers. It’s an exciting time filled with changes, including a health care transition.
A HEALTH CARE TRANSITION IS THE PROCESS OF GETTING READY FOR HEALTH CARE AS AN ADULT.
As a parent, you can do a few things with your child to make this transition successful.
- As your child approaches age 18, help them look for a new adult Primary Care Provider (PCP). Most pediatricians only see patients up to age 18. Log in to the Member Portal to select a new adult PCP.
- Discuss medical conditions, prescriptions, and health history with your child to help them feel more empowered to understand and discuss their health.
- Help your child make an appointment for a no-cost wellness checkup. Checkups continue to be a great opportunity for adults to monitor and treat existing conditions, get recommended adult vaccines, get age-appropriate screenings (blood pressure, weight, mental health)
