Please complete the following form to register for a Financial Wellness class presented by Credit Human in partnership with Community First Health Plans. You may register for more than one class.

To register, you must be a Community First Member or employee.

Please note, some classes are virtual, and some are in-person. If you're attending virtually, you will need internet access. A link to join the class will be sent to the email address you provide. If you are attending in-person, classes will be held at our Avenida Guadalupe Community Office located at 1410 Guadalupe Street, Ste 222, San Antonio, TX 78207. Lunch will be provided at in-person classes.
