Director, Population Health Management – service coordination

Community First Health Plans is currently searching for a Director of Population Health Management for the STAR+PLUS Program. The director will provide oversight and management of the Community First Health Plans, Inc. (Community First) clinical activities and deliverables related to utilization management (UM), service coordination (SC) and behavioral health (BH) assuring all contractual, regulatory and accreditation standards are met. They will also provide administrative and clinical guidance to all Population Health Management (PHM) clinical and administrative staff as assigned. The director will oversee the efficient utilization of resources within the schedule of benefits for Long-term Services and Support (LTSS) & acute programs for the line of business (LOB).

The ideal candidate would have worked three or more years in Managed Care with experience in service coordination, medical and behavioral health utilization management, medical record review, discharge planning, data analysis, and case management of acute and chronic illness. An understanding of long-term support services and service coordination for a disabled population is required.

Executive director, information systems

Community First Health Plans is currently searching for an Executive Director of Information Systems that will provide strategic support for all information technology, security, and configuration functions in the organization.  They will manage the design, planning, and implementation of all short and long-term strategies related to information technology and configuration. They will also ensure alignment of technology and configuration services to the business goals. Candidates must possess a high level of skills in design, development, analysis, and project management.

Member Service Representative

Community First Health Plans is currently searching for individuals who are passionate about helping others in our community to fill the role of Member Service Representative in our call center. As a Member Service Representative, your responsibilities would include helping our Members access and understand their health care benefits.

Provider operations coordinator

Community First Health Plans is currently searching for an experienced individual dedicated to improving the efficiency of our Provider network through recruitment, contract management, training, audits, and resolution and trending of Provider issues. The ideal candidate would serve as a lead in network development initiatives, communicating regularly with our valued Providers and leadership, so that we can facilitate a better overall Provider and Member experience while ensuring compliance with regulatory requirements.
