Non-contracted providers may use this Letter of Interest (LOI) form to express interest in joining the Community First network. We are currently accepting inquiries for network participation for PCPs and many specialties.

Instructions for Joining an Existing Group
If you are a provider who is not credentialed with Community First Health Plans and will be joining an existing contracted group in our network, please email dl_credentialingapplications@cfhp.com with the following information:
- Name
- NPI
- CAQH number
- Name, tax ID, and NPI for the practice/group you are joining
Instructions for Joining as a New Group
Community First Health Plans (Community First) continuously monitors and evaluates the availability and access to health care services provided in our network. Community First’s ongoing assessment of its provider network for availability and access standards ensures adequate services throughout the network. If you are interested in being considered for the Community First provider network, please complete the following form for each group/practice that is being submitted for consideration. Please allow up to 60 days for your application to be reviewed.
