For Community First Health Plans Prenatal/Postpartum Care STAR Medicaid Providers

Do you have a new Community First maternity patient?

Beginning January 1, 2024, Community First Providers who see new, pregnant Community First Health Plans STAR Members have the opportunity to receive an award of up to $80, in addition to their regular service fees. This supplementary compensation includes a designated $25 for the completion of the online Notice of Pregnancy (NOP) form below.

To qualify, the NOP form must be completed and submitted electronically within 60 days from the date of the initial prenatal visit when a Member is first enrolled with Community First. (Faxed submissions do not qualify for this program.)
