As of Aug 9, 2022, Community First Health Plans will implement a speech therapy policy.
There is now no pre-authorization required for speech therapy evaluations.
Treating agencies will need to submit a signed PT/OT/ST prior authorization form (see attached form) when submitting for visits.
For Members under 3 years of age, an Early Childhood Intervention (ECI) referral will need to be submitted with a request for visits.
Hearing testing needs to be completed per the TX Health Steps periodicity schedule (https://www.hhs.texas.gov/providers/health-services-providers/texas-health-steps/medical-providers)
