2026 Annual Member Survey

Attention, Community First Members! You might have received, or will soon receive, a survey in the mail about your experience with your health plan. 

Your voice matters, and your feedback helps us improve your care. We’d love to hear how we are doing.

Only some Community First Members are selected to receive this survey. It’s important that if you’re selected, you complete it as soon as possible.

This survey asks questions like:

  • How often does your or your child’s doctor listen carefully to you?
  • How often does your or your child’s doctor explain things in a way that is easy to understand?
  • How often does your or your child’s doctor show respect for what you had to say?
  • Is it easy to get the care/tests/treatment you or your child needs?
  • Are you able to get appointments with a specialist as soon as needed?
  • Are you or your child able to get needed care right away?

MORE INFORMATION ABOUT THE CAHPS MEMBER SURVEY

The Consumer Assessment of Healthcare Providers and Systems (CAHPS) is a survey of Member satisfaction. It is part of a national project by the National Committee for Quality Assurance (NCQA), a non-profit group that helps people learn more about health care plans. Conducting the CAHPS survey helps Community First meet NCQA accreditation standards and various state regulations.

The survey is in English and Spanish. A prepaid envelope will be included for you to return the survey via U.S. mail. Some Members may be given the option to complete this survey online. If your survey includes a QR code, scan it, and enter the username and password supplied to you.

Press Ganey, an independent research firm, is helping us conduct this survey. No one except the staff at SPH Analytics and NCQA will view your survey answers. Your answers will not have your name on them and will be part of a pool of information from others like you. If you have any questions, please call SPH Analytics toll-free at 1-888-797-3605.
