Community First Medicare Advantage Dual Eligible Special Needs Plan (HMO D-SNP) Members are invited to join our Enrollee Advisory Committee (EAC).
We need your ideas and experience about your health care plan to make it better for yourself and others.
Who can join:
- Community First Medicare Advantage D-SNP Members
- Authorized representatives, caregivers, and advocates for those Members.
What to expect:
- Rewards for eligible Members and participants.
- Educational topics related to your health plan.
- The opportunity to ask questions about your D-SNP health care plan.
- Sharing ideas on how to improve our services, access to services, service coordination, and the Member experience.
- Open conversations with Community First staff and other Community First Medicare Advantage D-SNP Members.
Meeting Information:
Meetings are held quarterly. Members can decide if they would prefer to attend virtually or in person. Below is the schedule for 2024.
Quarter 1: March 6, 2024
Quarter 2: June 5, 2024
Quarter 3: September 4, 2024
Quarter 4: November 6, 2024
