Locally Owned Health Plan Recognized for Excellence in Patient Outcomes and Experience
SAN ANTONIO, TX (November 13, 2024)- Community First Health Plans, the only locally owned and managed
nonprofit health plan in the Bexar Service Delivery Area, is proud to announce its recognition by
the National Committee for Quality Assurance (NCQA) with a 4-star rating in the commercial
health plan category. This rating is a testament to Community First’s unwavering commitment to
improving the health and well-being of its Members through comprehensive, community-focused
care.
The NCQA, a leading organization dedicated to improving health care quality, released its 2024
Health Plan Ratings on September 15. These ratings assess over 1,000 health plans nationwide
based on nearly 50 measures, including patient outcomes, quality of care, and patient
experience. Of the commercial health plans rated, Community First Health Plans earned an
impressive 4 stars, reflecting its dedication to providing exceptional care to the San Antonio
community.
While only three commercial health plans – Kaiser Foundation Health Plan of the Mid-Atlantic
States, Blue Cross and Blue Shield of Massachusetts, and Independent Health – received the
top 5-star rating, Community First is proud to be the only plan in Texas to receive this accolade.

Community-Driven Care
Established in 1995 by University Health, Community First Health Plans has been a cornerstone
of health care in South Texas. The organization serves Members across Bexar County and
seven surrounding counties, providing a wide range of health care services for newborns,
children, adults, seniors, expectant mothers, and individuals with disabilities.
Community First’s involvement in the community goes beyond traditional health care coverage.
The health plan actively supports local initiatives, offers health and wellness programs, and
engages with Members through in-person resources such as their Avenida Guadalupe
Community Office. Community First also offers a variety of Value-Added Services to its
Members, including programs for diabetes management, home-delivered meals, and fitness
opportunities like Zumba classes.

As an NCQA-accredited health plan, Community First Health Plans meets the highest standards
for health care quality and service. This 4-star rating is a direct reflection of the organization’s
commitment to patient-centered care, particularly in ensuring that Members have access to toptier
services at every stage of life. Community First also recently expanded its offerings with the
STAR+PLUS program, providing specialized care to adults with special needs across Bexar
County and surrounding areas, further demonstrating its focus on closing gaps in care.
