Community First Health Plans, Inc. is a non-profit HMO sponsored by the Bexar County Hospital District d/b/a University Health System. Community First’s principal business is the arrangement and provision of health care services for its members in accordance with the provisions of Chapter 843 of the Texas Insurance Code. Community First’s current service area includes the following Texas counties: Atascosa, Bandera, Bexar, Comal, Guadalupe, Kendall, Kerr, Medina and Wilson.
Pursuant to Subsection 11.1402 of the Texas Administrative Code and §843.305 of the Texas Insurance Code, this serves as public notice of the 20-day application period from January 3, 2020 through January 22, 2020, during which Community First Health Plans, Inc. invites physicians and providers in the geographic service area to fill out an application to participate in providing health care services or medical care under the terms and conditions established by Community First for the provision of such services and the designation of such physicians and providers. All requests should be submitted in writing to the above address.
