STAR Medicaid
A Medicaid health care program that covers children and adults (ages 20 and younger) who have disabilities.
STAR Kids
A Medicaid health care program that covers children and adults (ages 20 and younger) who have disabilities.
CHIP/CHIP Perinate
CHIP is a health care program for children (ages 18 and younger) whose families earn too much to get Medicaid but cannot afford private health insurance. CHIP Perinate covers unborn children and pregnant women who do not qualify for Medicaid and do not have any other health coverage.
Medicare Advantage (HMO)
A comprehensive Medicare plan that offers seniors (ages 65 and up) hospital coverage (Part A), medical coverage (Part B), prescription drug coverage (Part D), and extra coverage for things like vision, hearing, and dental.
Medicare Advantage (HMO) D-SNP
A type of Medicare Advantage (HMO) Dual Special Needs Plan (D-SNP) plan designed to provide extra help and support for people with low incomes or certain conditions and diseases.
University Family Care Plan
A health care plan for University Health employees.
HMO
Health care benefits provided through an employer including preventive care, emergency room visits, hospital stays, prescription drugs, and more.
University Community Care Plan
A high quality health care plan for individuals and families.
Provider Portal
As a Community First Health Plans Provider, you have access to an interactive, secure portal containing important information, tools, and resources. You can navigate the portal online or through our mobile app. Use the Provider Portal to:
- Verify Member eligibility
- Review Care Plans and Individualized Service Plans
- Submit prior-authorization requests
- Access forms and procedure documents specific to each health plan
