Community First Health Plans is working with Change Healthcare to help our members get the benefits they need. Change Healthcare will be helping our members who may receive Supplemental Security Income (SSI) benefits. If you are a member and receive a letter or phone call from Change Healthcare, please call us today to see if you can get these benefits at 1-844-735-6332 (TTY 1-888-987-2325).
