Community First Insurance Plans is accepting applications from physicians and providers who wish to participate in its EPO network. Applicants must meet the practice standards, qualifications, and other participatory criteria established by Community First. Providers can request network participation in writing or by completing a Letter of Interest online at CommunityFirstHealthPlans.com/Providers. Providers can apply at any time. Community First Insurance Plans is located at 12238 Silicon Drive, Suite 100, San Antonio, TX 78249. The Community First service area is Bexar County. Community First is the only locally owned and managed non-profit health maintenance organization in its service area and is sponsored by the Bexar County Hospital District, d/b/a University Health.
