Older adults and adults with disabilities who qualify for STAR+PLUS coverage will have access to Community First’s comprehensive care in the Bexar County Service Delivery Area effective September 1, 2024.
SAN ANTONIO, TX (February 26, 2024) –Beginning September 1, Community First Health Plans, Inc. (Community First) will be adding STAR+PLUS to its line of health care products. The Texas Health and Human Services Commission (HHSC) has selected Community First to administer the Medicaid managed care program STAR+PLUS in the Bexar Service Delivery Area, comprised of seven counties: Atascosa, Bandera, Bexar, Comal, Guadalupe, Kendall, Medina, and Wilson.
The STAR+PLUS Medicaid managed care program integrates the delivery of acute care services and long-term services and supports for seniors and adults with disabilities. As the only locally owned and managed non-profit health plan in the Bexar Service Delivery Area, Community First is proud to bring its high standard of care and commitment to the STAR+PLUS-eligible population.

Community First will offer STAR+PLUS Members all benefits under Medicaid, long-term services and supports, plus Value-Added Services*, including:
- Up to 8 hours of in-home respite care services
- Gift card rewards for Members who join Community First Health & Wellness Programs and complete required education
- YMCA Diabetes Prevention and Weight Loss Program with complimentary YMCA Membership
- Foot care and insulated cooler bag for Members with diabetes
- Mask with aerosol chamber and more products for Members with asthma
- Allowance for seat attachment that fits on a walker
- Enhanced vision services, including up to $125 for frames or $75 for contact lenses
- Low-cost dental services
- Home-delivered prepared meals after a hospital stay
- Baby Shower, baby items, and gift card reward program for pregnant Members
- Prescription Savings Card approved for use by uninsured family members
- Zumba classes for Members and their families with fitness giveaways
- Smartphone with two plan options
- Help getting rides to places like the grocery store, health education classes, and more
Community First will also guide new STAR+PLUS Members transitioning from other MCOs currently serving the Bexar Service Delivery Area that will no longer provide coverage. Community First can provide in-person support for this population at the Avenida Guadalupe Community Office located at 1410 Guadalupe Street, Suite 222.
HHSC will let STAR+PLUS members in the Bexar Service Delivery Area know if and when they need to choose a new health plan. Current STAR+PLUS Members should:
- Check their mail for information from HHSC and respond accordingly.
