Attention, Community First Members! You may have received a survey or will soon receive a survey in the mail about your experience with your health plan.

 Only some Community First Members are selected to receive this survey. It’s important that if you’re selected, that you complete it as soon as possible.

This survey asks questions like:

  •  Is it easy to get the care/tests/treatment you or your child needs?
  • Are you able to get appointments for yourself/your child with a specialist as soon as needed?
  • Are you or your child about to get needed care right away?

    MORE INFORMATION ABOUT THE CAHPS MEMBER SURVEY

    The Consumer Assessment of Healthcare Providers and Systems (CAHPS) is a survey of Member satisfaction. It is part of a national project by the National Committee for Quality Assurance (NCQA), a non-profit group that helps people learn more about health care plans. Conducting the CAHPS survey helps Community First meet NCQA accreditation standards and various state regulations.

    The survey is in English and Spanish. A prepaid envelope will be included for you to return the survey via U.S. mail or on the internet. To complete this survey online visit www.sphsurvey.com/plan or scan the QR code provided on the survey and enter the username and password found on the survey.

    Press Ganey, an independent research firm, is helping us conduct this survey. No one except the staff at SPH Analytics and NCQA will view your survey answers.
