The Centers for Medicare & Medicaid Services (CMS) requires all Providers who care for Community First Medicare Advantage D-SNP (HMO) Members to complete annual Model of Care (MOC) training and attest to its completion.
To meet the requirement, Community First Providers must complete our self-paced SNP MOC training course by December 31, 2025.
- Complete Community First MOC Training online or on the Community First Provider Portal.
- Submit Your Attestation. Complete the attestation form confirming you’ve reviewed and understand the MOC key components and your Provider responsibilities. If attesting for a provider group, please also attach a list of providers and NPI numbers.
