The Texas Health and Human Services Commission (HHSC) Provider Finance Department (PFD) is surveying AFC home providers in Texas to collect information for state fiscal year (SFY) 2023.
On Tuesday, September 3, 2024, the AFC SFY 2023 Survey will be available to download on the PFD Long-term Services and Supports (LTSS) website.
Download the template (.xslx) to submit the AFC Provider Survey. All completed surveys are due via email by 11:59 p.m. CDT on Friday, November 1, 2024.
Please email all completed cost surveys to HHSC PFD at PFD-LTSS@hhs.texas.gov.
- What is the purpose of the AFC survey?
The purpose of the survey is to better understand AFC in Texas. The survey responses will inform a review of the AFC reimbursement rate methodology and payment rates. HHSC is collecting the survey information in accordance with HHSC’s Biennial Fee Review, which requires HHSC to review all reimbursement methodologies and rates on a biennial basis. - Are all AFC providers expected to participate in the survey?
Yes, all AFC home providers that delivered services during SFY 2023 are expected to participate.
